A plan the whole team can read and keep.
Each plan sets out goals with their interventions beneath them. A review schedule shows when the next review is due, and the consent record is drawn from the resident's own assessment. The plan is signed off when it is complete, and the register shows which plans are not.
Goals and interventions
Each goal has its interventions beneath it, a record of progress notes and a Mark achieved step. An intervention records its own consent and links to the risk assessment it addresses.
Reviews on a schedule
Reviews default to every thirty days and can be set anywhere from seven days to a year. The register shows what is overdue and what is due this week, and a review is recorded with an outcome.
Consent that is read, not assumed
The plan cites the resident's consent record and states the status of that consent. A consent the resident has withdrawn is shown as withdrawn on the plan and in the register.

Add a goal, place its interventions beneath it, record progress notes and mark the goal achieved once it is met. Anything the plan still needs before sign-off is listed at the top.

Reviews and contingency

Each plan has a review schedule with a next date and reviews recorded with an outcome. One current contingency plan sets out the triggers, the signs to watch for and who responds.
