Eight tools ready to use
The eight are MUST, Waterlow, falls, manual handling, choking, infection control, safeguarding and a fire evacuation plan. Duplicate a core tool as your own and edit the questions.
Four stages to sign-off
The stages are identify, assess the impact, plan the mitigation and review the effectiveness. They are completed in order, and sign-off is not possible until consent is recorded and a medium or high risk has a linked care plan intervention.
Overdue reviews listed on the register
Overdue reviews are shown at the top, with a panel listing residents with no assessment on record and filters for status, risk level and review. A count is shown only when it is above zero.

What sign-off requires
Sign-off requires all four stages complete, consent recorded, the Mental Capacity Act chain where capacity is lacking and a linked care plan intervention for a medium or high risk. The record is then locked, and the link cannot be removed.
